HOUSING CASE MANAGER – CO-OCCURING DISORDER SPECIALIST

Social Services Program Mgr.

             Status:

Exempt/Full Time

 

 

Hours:

Flexible 40 hour work week.  Schedule includes work one late night (8 PM).  Flexible scheduling with 3 other team members. 

 

     Compensation:

$32,000 - $36,000 DOQ plus benefits, including medical, dental & vision insurance, long-term disability, group life insurance, 403(B) retirement plan, PTO & holiday, bus pass subsidy & EAP

 

AGENCY OVERVIEW

Plymouth Housing Group owns and manages 11 buildings that provide nearly 800 units of permanent housing for homeless or very low-income people in downtown Seattle. PHG cannot end homelessness simply by providing housing. Many homeless people need much more than a roof over their heads—they need medical care, mental health counseling, chemical dependency services, and life-skills coaching. They may also benefit from creating healthy connections within their community. Our goal is to assist formerly homeless and very poor tenants to establish a stable life in permanent housing by providing needed resources and teaching basic life skills, thereby empowering tenants to manage their lives effectively.

 

JOB SUMMARY

This position is responsible for the delivery of homelessness prevention and housing stabilization services to tenants 55 years and older who were formerly homeless with co-occurring mental health and substance use disorders. Work will include establishing strong relationships with tenants, creating community within the building, and developing partnerships with community service providers. This specialized work supports a tenant’s creation of home via personal goal setting, behavioral change, harm reduction, service referrals, and life and relationship skills coaching.  The position will also serve as a training and consultative resource for the Social Services team.

 

ESSENTIAL JOB QUALIFICATIONS:

(Any equivalent Combination of Knowledge, Skills, Abilities, Education, and Experience)

1. Education: Minimum of BA/BS degree in a human services field.  MA/MSW preferred.  Advanced education can be substituted for experience and/or years of experience can be substituted for the required degree.

2.  Ability to maintain working knowledge of diagnostic criteria presented in the DSM. Demonstrate the ability to discriminate between overlapping diagnoses of mental illness, acute and chronic substance abuse related behaviors and related mental status changes.

3.  License/Certification:  Valid Washington State Driver’s License

4.  Excellent written and verbal communication skills.  Group training and facilitation experience preferred

5.  Ability to be flexible, supportive and to work cooperatively with staff and outside agencies.

6.  Ability to utilize computer technology efficiently and effectively including the internet, Microsoft Word and Excel, databases.
